A Note from the Nurse

Dear Parent or Guardian of a New RMS Student,

Welcome to Rundlett! You child's heath is important to us. We look forward to working with you and your child. Please call our office with any questions or concerns you may have. Our direct line is 225-0868 and we can be reached by email at alanney@sau8.org.

DOCUMENTS required for all incoming students:

-Please make sure we receive a recent physical exam report and an updated immunization record that contains the Tdap and second Varicella vaccines requires for all middle school students age 11 or older.

-All students are required by the state of New Hampshire to have an up to date immunization record or religious exemption form on file with the nurse

MEDICATIONS:

-If your child will be taking prescription medication at school, we need written permission from the prescribing health care provider and a parent/guardian. The note must clearly state the name of the medication, dosage, time and diagnosis. All medication must be brought to the health office in the original pharmacy container. Students are not allowed to carry medication during the school day. There are some exceptions such as inhalers, epipens, or insulin, but details should be addressed with the nurse first.

-We stock over the counter medication for students: acetaminophen (generic Tylenol), ibuprophen (generic Motrin or Advil), TUMS, Sudafed PE, Benadryl and hydrocortisone and antibiotic ointment. Please make sure you fill out Section 11 in the online enrollment form. This is vital information we will need in the health office to care for your child and to get in touch with you. The form contains the OTC medication checklist to give us permission. Please check off what is appropriate to give to your child.

HEALTH CONCERNS AND ALLERGIES:

-If your child has a severe allergy to an epipen or asthma requiring an inhaler or nebulizer, please contact the nurse for the med permit and field trip forms and to discuss how we can help your child address these issues in school.

-If your child has chronic illness, please contact the nurse so we can address meeting your child's needs here at school.

Remind your child that we are here if they feel ill, are injured, nervous or just need some friendly assistance with a problem. We are looking forward to a great school year!

Sincerely,

Ann Lanney, RN and Erin Stewart, RN
